Our Adult Day Health Care is a medical model adult day care program that serves adults with special physical, mental and psychosocial needs. Members are encouraged by staff to increase their self-esteem, social interactions and independence. Members who participate in our Adult Day Health Care Program will be able to remain with their caregivers and strengthen ties to their community.

Our ADH program is capable of serving up to 72 members each day 9:00AM to 4:00PM Monday to Friday. Our staff is caring and will treat members like their family. We also welcome those who seek activities and social interaction with peers.

All individual's care planning is very important to us! We begin the process by completing a comprehensive assessment of the individual’s medical and mental health history in order to provide the best care possible. Once the individual is approved and attending our program, our nursing staff will complete a care plan based on the individual’s medical and mental diagnosis in order to ensure that Families Together staff help the individual achieve his or her goals.

Our agency also offers other services such as in-home individual counseling. ADH members can take advantage of additional programs to stimulate their mind and body.

The ADH course of service includes:
• Assistance with daily medication
• Assistance with personal care and hygiene
• Nursing
• Physical and occupational therapies
• Delicious breakfast and lunch
• Daily Activities (knitting, painting, arts and crafts, soft yoga, walking groups field trips, beauty parlors, and (biblical studies) for those interested)
• Transportation will be provided.
